HIPAA Notice of Privacy
Practices
The federal Health Insurance Portability and Accountability Act (HIPAA)
requires most health plans and health care providers to issue a Notice of
Privacy Practices to the people they serve. The Notice describes how
individual health information is used and disclosed. It also describes
rights individuals have such as accessing or requesting changes to their
health records, or receiving an accounting of where their information has
